What to Bring to Your First Dental Visit
To help you get started, you can fill out the relevant forms on our website and at your leisure. No need to print!
At your first visit, bring the following items or information:
- Any recent films or oral health information
- Your dental insurance card
- Any new or relevant health information
- Your copay or service fee
If you don't have dental insurance, we have our own Kalaheo Dental Group Membership Plan. This is not dental insurance but a plan allowing you to get the preventive dentistry you need to keep your smile healthy. For a low annual fee, the dentist provides the following treatments yearly:
- 2 Teeth cleanings
- 2 Dental exams
- 1 Set of x-rays
- 2 Fluoride treatments (for children 0-18)
- 20% Discount off select treatments and services

Your dentist near me is located on Kaumualii Hwy, between Silva Tract and Ganansa Tract Roads, just across from the Yoga Garage and Maranatha Church of God. Our office is next to H&R Block. Most patients take Kaumualii Hwy all the way to our dental office.
